CU 125 Form - Please click HERE to start the CU 125/Oath of Directors application. After you complete the application, you will be able to view or print the report.
Please Note: If you only wish to print the signature page (page 2) at this time and complete the report at a later date – to do so you will need to tab through the report and, at a minimum, enter the dates of the Annual & Organizational meeting and then click “next” to save the data. If you do not know the exact dates, you can put in any date, print page 2 and then correct the dates once your Annual & Organizational meetings have concluded and you finalize the report.
The CU 125/Oath of Directors (electronic and hard copy (or hard copy only if completing manually) must be finalized and submitted within 30 days of your Organizational Meeting.
